The PHQ9 Questionnaire is used by GP’s to asses your depression. If you suspect that you or, someone you know, has depression have a look at the form by clicking the link to load it and answer the questions HONESTLY.
It is only a starting point but if your answering “More than Half” and “Nearly Every Day” to most of the questions then you need to see your Doctor.
No crime in having depression, it is not a sign of weakness so don’t be afraid to talk to someone about it!
PHQ-9 Depression Severity
This is calculated by assigning scores of 0, 1, 2, and 3, to the response categories of “not at all,” “several days,” “more than half the days,” and “nearly every day,” respectively. PHQ-9 total score for the nine items ranges from 0 to 27. In the above case, the PHQ-9 depression severity score is 16 (3 items scored 1, 2 items scored 2, and 3 items scored 3). Scores of 5, 10, 15, and 20 represent cutpoints for mild, moderate, moderately severe and severe depression, respectively. Sensitivity to change has also been confirmed.
This quote resonates for me: “Depression is not a sign of weakness. It is a sign that you have been trying to be strong for too long.” It’s not good to bury feelings and we should be able to ask for help as well as give it.

Ah i’ve filled that one in quite a few times, and the GAD 7. I’m probably due another for another appointment. Great idea to have a separate page with this on.
